Who signs this form?
First, it is important to refer to the online forms section of the SFSU main website for information regarding the signature requirements for each form. We strongly encourage you to meet with the instructor whenever a form needs their signature. Forms that require the signature of your department chair/director can be left in our office and in many cases we will handle the form from that point on. Forms requiring the signature of the Dean should be forwarded to the Office of the Dean of BSS in HSS 359, and are generally forwarded by the office staff.
When is the last day to register for classes, drop without receiving a W, etc.?

How do I register for Individual Study courses in CIP?
You will need to complete the "Petition for Course by Individual Study," being certain to discuss the objectives of your study with an instructor and your advisor, both of whom must sign the petition. Once the petition is fully completed and you have obtained all signatures, you can bring it to the CIP office located in HSS 336 where we will forward it to the appropriate program chair/director. Be sure to include either your e-mail address or phone number on the form so we can provide you with registration information once we have received approval from your department chair/director.
